Patient Appreciation Giveaway

Sullivan Heights Dental gives away a great prize every few months to one of our current patients.

Be sure to check our Facebook page often to see what we’re giving away next. Please note: this contest is in no way sponsored or administered by Facebook.

How Do I Enter?

Enter in person at our office by filling out a ballot. Even if you do not have an appointment scheduled, you are welcome to come in to enter the draw!

After the stated deadline for entry, our staff will perform a random draw for the prize. The winner’s name will be announced on our Facebook page, and he/she will also be contacted by the clinic. Be sure to watch our Facebook page feed closely.

If you win, you must claim your prize within two days (48 hours). If we don’t hear from you within that timeline, a new prize winner’s name will be drawn. You will be responsible for promptly picking up your prize from the clinic – we are unable to ship, deliver or hold your prize for a period of time.

Note: There is a limit on winning of once every 6 months so that everyone gets a chance; and only one entry allowed per family.

Referral Rewards

One of the greatest compliments a patient can pay us is referring others to our dental clinic.

As a new practice, we depend on happy patients and word of mouth. It would be greatly appreciated if you could help us by telling your friends, family and coworkers about our practice.

For each referral that you send us, we’ll thank you with a $25 gift card.

How Does Referral Rewards Work?

Tell your family, friends and/or coworkers about Sullivan Heights Dental.
Upon making their first appointment, they will be asked by our receptionist how they heard about the clinic and who referred them.
After their appointment is finished, your name will be added to our Referral Rewards list. At the end of each month, we will contact everyone on our Referral Rewards list, so be sure that your contact information remains up to date in our system.

Note: To qualify for a referral reward, you must be an active patient with our clinic, i.e. your name and address is in our database, and you’ve come in for an appointment in the past 12 months.
