Patient Appreciation Giveaway Rules

SULLIVAN HEIGHTS PATIENT APPRECIATION
GIVEAWAY CONTEST

CONTEST (“CONTEST”) ADMINISTERED AND SPONSORED BY SULLIVAN HEIGHTS DENTAL (“SPONSOR”). CONTEST BEGINS ON THE FIRST DAY IT IS ANNOUNCED ON THE SPONSOR’S FACEBOOK PAGE AND ENDS ON THE STATED DEADLINE AT 5 pm PST. (“CONTEST PERIOD”).

NO PURCHASE NECESSARY TO ENTER OR WIN. Only a BALLOT ENTRY is required. By participating, entrants agree to be bound by these Official Rules and the decisions of the Sponsor and Administrator. Void where prohibited by law.

Contest is open to legal residents of the Greater Vancouver Regional District [see map] who are existing patients of the Sponsor. Employees or contractors of the Sponsor(s) and immediate family members of, and any persons domiciled with any such employees or contractors, are not eligible to enter.

One winner will be chosen at random for each contest period. The odds of winning will depend on the number of entries received. The winner will be notified by phone, email and/or through direct message on Facebook, and will have forty-eight hours to respond. If no response is received within forty-eight hours, another winner will be chosen.

Sponsor is not responsible for prize fulfillment nor for any technical failures. Winner must pick up prize or make arrangements to pick up prize within 72 hours of being notified of the win, from Sullivan Heights Dental in Surrey, British Columbia. Winning entry will be verified. The winner’s name and photo will be displayed on Sullivan Height Dental’s Facebook page.

A reply to the winning email or phone call is required within 48 hours before a new winner is chosen.

Only one entry per patient, per family and per prize draw. A prize winner may not enter the contest again until six months have passed from the date he/she won the monthly giveaway prize.

Residents of Canada are required to enter a skill-testing question in order to claim their prize.

Sponsor reserves the right to publish winner’s name.
